How It Works

Step 1: Patient Data Collection

C365 collects a detailed record of each resident’s health history along with medical, social, family and professional resources, as well as detailed contact information and protocols for each. This information is collected in a variety of forms and is then committed to the C365 database in a secure, HIPAA-compliant format.

Step 2: Daily Check Ups

Each day, residents sit down at the kiosk, which securely identifies each user before beginning each session. The kiosk user interface walks the user through a series of questions that can be tailored to each resident, gathering valuable information about health and vital signs, and delivering the information to the Connections365 Health Monitoring Team.

Step 3: Continuous Monitoring and Healthcare Referral

By regularly monitoring this information, the Connections365 Health Monitoring Team can recognize changes in each resident’s condition and take action. From reminders to take their medication, to contacting family or medical resources about areas of concern, to referring residents to the appropriate health care providers when needed, our Health Monitoring Team provides the human decision-making element you want in any health monitoring system.
